You can get to know if you are using Legacy Product Catalog or Latest Product Catalog by checking these options. Once you know which version you are using then we can check the URL format.

URL format for Latest Product Catalog

If you are using Latest Product Catalog then you checkout URL should be similar to - https://{sitename}.chargebee.com/hosted_pages/checkout?subscription_items[item_price_id][0]={plan-id}
